After few test it's appear that poison damage are base on hits of the npc.

The problem is if we have a big Monster of 20000 hits. Damage is too heavy.

It's possible to apply damage base on str or ostr?
If yes I can do it in script?

The old formula always cause the damage based on % of maxhits, that's weird, but its how the old formula works

To change this you must enable MAGICF_OSIFORMULAS on sphere.ini (this will make all spells use OSI formulas) or replace the poison spell effect with a custom effect using a custom memory
